Casale and Nocetum appear in medieval documents as two separate places under different lords: in 972 Otto I confirmed Nocetum to the abbey of Bobbio and the same year named Casale in a grant to a monastery in Pavia. Old Nocetum lay along a through road, at what is now Casale vecchio.
In 1373 the village, Ghibelline against Guelph Tortona, was levelled by Giovanni Acuto's troops and rebuilt on its present site by order of Galeazzo Visconti. One fourteenth-century tower of the old walls survives, and it is no longer a tower: the church of San Rocco sits inside it, its vault frescoed by the Genoese Lazzaro Tavarone.
