San Sebastiano da Po lies where the Leona valley opens onto the plain. A document of 1278 names it beside Radicata, a neighbour it later absorbed. The Po then ran at the foot of the hills; embankments forced it into its present course. The crossing was shared with Radicata, a fief of Chivasso, whose men paid no ferry toll.
In the sixteenth century the village still had crumbling walls and two gates, and above them a tower and an old castle, pulled down in the seventeen hundreds for a palazzo. The communal oven had fallen in, many families having moved out to the hamlets, and the parish church stood on the square.
