The patron saints of Cava de' Tirreni are Our Lady Crowned of the Elm and Saint Adiutor, celebrated on 8 September: the solemn feast has been held on this date, the feast of the Nativity of Mary, since 1581, by decree of Bishop Alemagna de Cardona. In July 1931 Pope Pius XI raised the church dedicated to her to the dignity of minor basilica, recognising its importance to the town's devotion.
Among the most important religious and folk events of Cava de' Tirreni is also the celebration in honour of the Blessed Sacrament, known as the Festa di Montecastello: held every year since 1656, the year it originated, in memory of an event that involved the town community at that time. The town also holds the Abbey of the Most Holy Trinity, among the area's main places of worship, and the Castello di Sant'Adiutore, testifying to a religious and historical heritage built up over the centuries.
