Millesimo knows its own birthday: 9 November 1206, the day of the walls. Enrico II Del Carretto had them raised together with the castle, right where the Gaietta bridge crosses the Bormida: a strategic spot and, above all, a toll. Beyond the bridge the strada magistra langarum climbed towards Cortemilia; on this side you could go down to the port of Finale and avoid the Genoese customs.
The village is now one of the most beautiful in Italy. On the night of 16 to 17 August 1809 the town hall lodged pope Pius VII, Napoleon's prisoner. The population is 3,426.

On 16 July Millesimo celebrates the Madonna del Carmine; the town, walled by the Del Carretto marquises, was the scene in 1796 of the Battle of Cosseria, opened by Napoleon Bonaparte.
