Above the village lies Campitello Matese, and the Giro d'Italia has finished up there four times: in 1988 Franco Chioccioli won, in 1994 the Russian Evgenij Berzin, in 2002 Gilberto Simoni, and again in 2015. In winter people ski, in summer they come up for the cool air.
The name comes from San Massimo, bishop of Nola: his followers, fleeing the persecutions of 251, are said to have stopped in these parts. The village first appears in writing in 1113, in the surname of a Norman, De Sancto Maximo; of its castle, brought down by the 1805 earthquake along with the church, few stones are left. There are 836 residents.

San Massimo celebrates Saint Maximus of Nola on 15 February; his bust also appears on the municipal coat of arms, while the village, on the eastern slopes of the Matese, is a winter tourist destination thanks to the Campitello Matese hamlet, more than once a stage finish of the Giro d'Italia.
