The patron saint of Roccamandolfi is Saint James the Greater, celebrated on 25 July, in the church dedicated to him. In 1780, Duchess Anna Pignatelli obtained from Pope Pius VI the body of Saint Liberatus the martyr, to be venerated precisely in the church of San Giacomo Maggiore: since then, the small shrine set into the church's apse has also become the sanctuary of San Liberato, so much so that today the whole sacred building bears both titles, «Sanctuary of San Liberato» and «Church of San Giacomo il Maggiore», thus housing both the relics of the holy apostle and the body of the martyr.
Saint Liberatus is an anonymous Roman martyr, venerated under this name at Roccamandolfi for over two hundred years: his feast, in which his remains are venerated, is held on the first Sunday of June, and once drew pilgrims from the whole Campanian and Molisan Matese, who visited the saint for the fifteen days before and the fifteen days after that date. Lordship over the town passed over the centuries through several families: the Gaetani held it as feudal lords in the first half of the 15th century, until 1456, when Giacomo Gaetani sold it to the Cennamo family, and later to the Perez family.
