The place is recorded as Trecius in 1171, and the name probably comes from Troecius, a Roman personal name. Tinella is a late addition, granted by royal decree in 1862, and notes that the commune sits at the springs of that stream.
In Roman times Trezzo was a junction on the via magistra Langarum, running from Alba to Cortemilia and the coastal towns; in 1879 digging for a railway never finished brought its course to light where it crossed the Tinella at the San Giovanni bridge. Of the castle that carries its whole medieval story nothing survives: the tower fell, the ruins were cleared. The present bell tower may have belonged to its precinct.
